Reablement Service (Joint Care Provider Service)

Working with people to regain independence within their own home and community

Sometimes, due to an accident, illness, or following a stay in hospital, people can struggle with their daily activities or feel unsafe at home. We believe that for many of these people, a short, intensive period of help and support can make a huge difference to their quality of life.

Our Reablement Service supports people to regain their independence so that they can remain in their own home and community with a minimum level of support.

How does the Reablement Service work?

The Reablement Service supports people coming home from hospital, and, as part of the Joint Care Provider Service, we will work with you to provide the extra support which may be needed.

The service is available for a period of between one and six weeks based on assessment of need. During this period, the Joint Care Provider Service will work with you, and those who care for you, to establish what ongoing care or support, if any, is needed. You will be given every opportunity to regain your independence and confidence in any daily activities that you are struggling with.
