Leaving Care Team

The 16+ and Care Leavers Team work with young people aged between 16 years and 21 years and up to 24 years in some cases depending on a needs assessment.
Social workers work with you to put arrangements in place for all 16 to 17 year old young people to help prepare them for leaving care when they are 18. We do this by listening to what you want for the future and helping you each step of the way - this can be practically, financially and emotionally. At 18 you will be allocated a Personal Adviser, who will support you to develop skills to become an independent adult who is in control of your own life..
