Support Leaving Hospital (Maximising Independence Team)

Helping adults to return home from hospital safely and in a timely way

Coming home from hospital can be a difficult and worrying process.  The Maximising Independence Team work to make sure that your return home from hospital is safe and timely.

We work closely with adult hospital leavers to help them adjust to new health and personal circumstances on their return home. Our first contact with you will usually be made before you leave hospital, to discuss your needs and find out about any extra support you need.

The Maximising Independence Team will:

  • work closely with hospital staff to make sure that your care and equipment needs are met when you are discharged home
  • provide you with a Hospital Link Worker as a first point of contact - these are experienced social care professionals working at each of the local NHS hospitals and will provide you and your family with information and support
  • arrange a Care Needs Assessment, if appropriate, to help us to understand whether you meet our eligibility criteria for further help

Joint Care Pathway

The Maximising Independence Team work with our colleagues in the Berkshire NHS Intermediate Care Team in order to provide the Joint Care Pathway (JCP). The JCP is a service which aims to enable you to regain a level of independence following hospital discharge. The service is free of charge for up to six weeks. The actual length of time you could remain with the service will be determined by an assessment of your needs once you have been discharged.

Information for hospital staff

Discharge paperwork, equipment and requests for home adaptations which are essential for discharge (including keysafes) should be faxed through to The Health Hub.
