Safeguarding Adults Reviews (SAR)

Learning safeguarding lessons for the future

Safeguarding Adult Reviews (SARs), make sure that the Safeguarding Adults Boards (SAB) are fully aware of any safeguarding failures, so that all the organisations involved can improve their practice. All members of the SAB must cooperate and contribute to the review, as stated in the Care Act 2014.

The West Berkshire Safeguarding Adults Board have developed guidance for Multi-Agency Reviews of Serious Cases in order to ensure that:

  • there are processes for learning and reviewing that are flexible, proportionate and open to challenge
  • we can decide what type of review is appropriate, taking into account the case and the agencies involved
  • a culture of transparency is created

All SARs completed by West Berkshire Council and other neighbouring authorities are published on the Safeguarding Adults Board website.

Learning Logs

We routinely collate published cases that are of interest and relevant to our area. They might include SARs that have been completed by other authorities, but provide lessons that should be shared more widely. 

We produce a quarterly Learning Log for providers, statutory partners and any other interested party. We've listed some of the logs below, but if you would like to see one that isn't listed below, please contact us.
