Reablement Service (HomeSafe / Home Improvement Service)

Working with people to regain independence within their own home and community

Sometimes due to an accident, illness or following a stay in hospital people can struggle with daily activities or feel unsafe at home. 

We believe that for many of these people a short, intensive period of help and support can make a huge difference to their quality of life. 
Our pdf icon HomeSafe and Improvement Care Service [752kb] supports people to regain their independence so they can remain in their own home and community with a minimum level of support.

The service is available for a period of up to 6 weeks, and has two aspects:

1. HomeSafe

Our HomeSafe scheme supports people coming home from hospital. 
We aim to get you home as quickly as possible by providing you with the extra support needed during those first few days.

2. Improvement Care

This service allows us to work with you and those who care for you for a specific period of time.  We establish what ongoing care/support, if any, you need.
You will be given every opportunity to regain your independence and confidence in any daily activities that you are struggling with. 

Our goal is for people to regain their independence so that they can remain independent in their own home with the minimum level of support.
